I don't know diddly, but i thought i read using ether to start a 440 icd is really a bad idea. The guy who I bought mine from specifically mentioned it as something to avoid unless I wanted to pay someone to rebuild my engine.

Having the injectors rebuilt cured my hard starting 440. Mine also has the lever next to the shifter but I don't have a clue what it does.

All transmissions were set up with the PTO leaver from factory the shaft was optional. The bottom three bolt cover on the rear of the transmission is for the 540 PTO shaft.

So is it as simple as finding a PTO shaft and sliding it in?

Yes, remove the cover and install the shaft. There is an internal oil seal on the power shaft that keeps the oil from coming out.

Almost no original Detroit 53 series running S series pistons will start much below 70 degrees without a whiff of ether. An ether injector was a factory option, seen them on the 435 wheel tractors years ago.
5 speed was an option, PTO was an option.
Remember to run only Delo 100 or equivalent low ash oil in a two stroke Detroit.
