Here is the machine serial number tag breakdown using the 350 Introduction from the JD 350 Parts Catalog:
SN = serial number
T = Tractor
4 = JD350
F= Crawler - Dozer
3 = Diesel
D= (Transmission) Sliding gear with reverser
Serial number: 096852 (This number falls in the 1969 build range using the serial number list on this site’s FAQ board)
T= (Build location) John Deere Dubuque Works
This serial number is a 1969 straight 350 crawler equipped with dozer blade having a diesel engine and a sliding gear transmission with a reverser.

Not sure where you will ever find a re-built reverser for one of these 350s. I highly recommend you check with Lavoy who owns this site first. I know he keeps a lot of new parts that will help you out. Otherwise there is PB&H out in New Hampshire and Zimmerman Tractor here in Missouri. You could also google for salvage parts near your location.
Your bearing being bad on the front quill is common. It is the usual suspect on any 350 reverser with issues and is a weak point in the design. Deere has modified the bearings and the quill itself over the years trying to correct the issue. Not sure they ever got it completely fixed since I did about as many 350D reversers as I did the old straight machines.
The cracked end on the shaft was probably caused when someone tried to remove that bearing without first removing the plug out of the end of the shaft. This usually happened when people tried to overhaul the reverser without a tech manual to refer to. It specifically says to remove that plug first.
Your broken tube is also caused by someone who tried to remove the clutch assembly without first removing the tube. They remove that spacer and think that's all there is. Then when they yank and pry and try to force the assembly out of the case it breaks the tube.
These reversers are easy to rebuild if you have a Tech Manual to refer to and also some basic tools. You will need a press to disassemble and reassemble the assembly. You will also need a dial indicator to set your preloads on the two shafts and their bearings. I think Lavoy will have all of your parts or at least most of them and I would sure call him first.

Will this be the same reverser as what is in a 350B or 350C?

I am also highly skeptical you will find a rebuilt reverser, just not enough demand for anyone to bother with any more.

Bearings and such are available, clutch packs are no problem, many hard parts are obsolete.
